Somewhere between the ice bath girls and the "hum for 30 seconds before meetings" crowd, wellness TikTok developed a new fixation: the vagus nerve. You've probably seen it by now. Someone dunks their face in a bowl of ice water and says their nervous system "reset." Someone else breathes very slowly into the camera and tells you their HRV went up 12 points. A creator you follow posts about "toning their vagus nerve" like it's a glute.
If you glazed over and kept scrolling, fair. But the actual science here is interesting enough that it's worth understanding before you dismiss it as another wellness micro-trend. Unlike tongue scraping and magnesium sprays, the vagus nerve research has been piling up in peer-reviewed journals for years, and some of it is genuinely surprising.
Your nervous system has two modes, and most of us are stuck in the wrong one
Your autonomic nervous system runs in the background managing everything you don't consciously control: your heart rate, your digestion, your breathing, the weird way your stomach drops when you get a stressful email at 11pm. It has two main settings. The sympathetic system is your fight-or-flight response: cortisol floods in, heart rate spikes, blood goes to your muscles, your body does everything it can to help you survive an immediate threat. The parasympathetic system is the opposite: "rest and digest," the mode where your body can actually recover, repair, and regulate.
The vagus nerve is the main cable of your parasympathetic system. The word comes from the Latin for "wandering," which is fitting, because this nerve genuinely wanders, from your brainstem down through your neck, through your chest, into your gut. (This is why stress lives in your stomach. Your vagus nerve is connecting all of it.) It's the longest cranial nerve in your body, and it's responsible for signaling your body to calm down after stress, regulating inflammation, and supporting digestion.
Vagal tone is basically a measure of how well all that is working. Think of it like cardiovascular fitness, but for your nervous system. High vagal tone means your body can shift smoothly between activation and recovery. Low vagal tone means the stress response lingers longer than it should. Your cortisol stays elevated. Your inflammation stays up. Your sleep suffers. And yes, your skin reflects it.
HRV, explained without a PhD
The reason people keep mentioning HRV (heart rate variability) alongside vagal tone is that HRV is essentially how we measure it. Here's the detail worth knowing: your heart doesn't beat like a metronome. The time between beats fluctuates slightly, and those fluctuations are actually a sign of good health. Higher variability means your nervous system is adaptable and responsive. Lower variability means it's rigid, stuck basically, and low HRV is associated with chronic stress, anxiety, inflammation, and burnout.
Your HRV drops when you're sleep-deprived or sick. It drops after a week of bad eating or too much alcohol. It rises when you're well-rested, getting regular movement, and doing things that activate your parasympathetic system consistently. You can track it with a wearable, and watching the number move based on your habits is one of the more illuminating health experiments you can run on yourself.
Does Slow Breathing Do Anything? Turns Out, Yes
Yes, and this is where the research gets good. Specifically, slow diaphragmatic breathing, around six breaths per minute with longer exhales than inhales, has measurable effects on vagal tone. A 2025 review published in the journal Stress and Health analyzed 30 studies and found that this style of breathing significantly improves HRV, reduces cortisol, and shifts the nervous system toward parasympathetic dominance. Even a single session makes a measurable difference. There's also a 2021 study in Scientific Reports that found one session of deep slow breathing improved vagal tone and reduced anxiety across multiple age groups.
The mechanism is actually elegant. When you inhale, your heart rate speeds up slightly. When you exhale, it slows down. Making your exhale longer than your inhale, breathing in for four counts and out for six or eight, essentially keeps your body in "slow" mode for longer on each cycle. One note from a researcher who studies this: Sébastien Laborde, a sports scientist who has spent years studying HRV, says that before any of the fancy interventions, the priorities are sleep, nutrition, and exercise. "Get those three right and then we can talk about stimulating your vagus nerve." Which is not the influencer answer, but it's the honest one.
Cold Water Splashing Sounds Unhinged. It Works.
When cold water contacts the skin on your face, specifically around your forehead, eyes, and nose, it triggers the mammalian dive reflex. This is an ancient survival mechanism that exists in every mammal. When your face detects cold water, your trigeminal nerve fires a signal to your brainstem, which activates your vagus nerve and pushes your body into oxygen-conservation mode: heart rate slows, breathing shifts, your system calms down.
A 2025 study published in the European Journal of Cardiovascular Medicine measured what happens when participants submerge their faces in water at different temperatures. In 10-degree cold water, heart rate dropped significantly in every participant, and middle-aged participants had an even more pronounced response than younger ones. That drop is direct evidence of increased vagal activity. The cold is doing something real.
Interestingly, doctors have been using this reflex clinically for decades. Cold water on the face is a documented, evidence-based treatment for a dangerous type of rapid heart rhythm called paroxysmal supraventricular tachycardia. The wellness creators didn't invent this. They just discovered that it's kind of pleasant to do before work. A 30-second cold water face splash in the morning isn't a gimmick. The trigeminal-vagal pathway is real, it's fast-acting, and it doesn't require a special device or a $400 plunge pool.
Why vagal tone shows up on your face
Chronically low vagal tone means your body spends more time in sympathetic overdrive than it should. And cortisol doesn't just make you feel bad. It actively degrades collagen, disrupts your skin's barrier function, increases inflammation, and throws off your sleep. Stressed skin is a real phenomenon, and the root of it is physiological, not cosmetic. You can buy every barrier repair product on the market and still look exhausted if your nervous system is running hot constantly.
High vagal tone, on the other hand, supports lower baseline inflammation, better sleep quality, more even cortisol patterns, and faster recovery from everything, including the environmental assaults your skin takes every day. It's not magic. It's more like infrastructure. Understanding the vagus nerve gives you a framework for why the habits that feel good actually work: better sleep, more movement, breathing exercises, cold water in the morning. None of it is random.
